Dear Parent/Guardian:
We would like to remind you of the policies that have been put in place to ensure the health and safety of children needing medications during the school day.  In order to comply with state laws, our school district requires that the following items must be on file before we begin to give any medication at school:
1.   Signed consent by the parent or guardian to give the medication. Please complete the enclosed “SAU 21 Permission to Administer Medicine in School” form and give it to your school nurse.
2.   Physician signed medication order. The enclosed form may be taken to your child’s physician for completion and returned to the school nurse.   Alternatively, your child’s physician may fax a copy of signed medication orders to the school.  This order must be renewed as needed and at the beginning of each academic year.
Medications should be delivered to the school in a pharmacy or manufacturer-labeled container by you or a responsible adult whom you designate. Please ask your pharmacy to provide separate bottles for school and home. No more than a thirty-day supply of the medicine should be delivered to the school. Students should not transport medications to school.
Please complete these steps as soon as possible to avoid a delay in your student receiving their medication.  Medication will not be given until both the parent consent and the physician’s signed medication orders are on file.  Thank you for your help.
